If you (the BENEFICIARIES) or (the AUTHORIZERS) are unable to complete this form, please call us at <a href=\"tel:450-977-4411\">450-977-4411<\/a><\/p><p>\u00a0<\/p><p>Once you&#8217;ve filled in the form, we&#8217;ll evaluate the services and associated costs. We will send you this information in writing, after which we will call you. Once available personnel have been identified, you should receive services within two hours of the original call.<\/p><p>\u00a0<\/p><p>Please note that we charge fixed prices which will increase by 3% on January 1st each year. These fees are:<\/p><ol><li>Medical file opening fee (charged only once, with the exception mentioned below)<\/li><li>Crisis management costs<\/li><li>Government rate per km<\/li><li>Hourly rate for attendant\/nurse* (AGENTS), for a minimum of 2 hours.<\/li><\/ol><p>\u00a0<\/p><p>Note that you can choose up to 14 slots per week of continuous service, i.e. 2 hours at a time. morning (a.m.) and 2 hours late afternoon (p.m.)<\/p><p>\u00a0<\/p><p><strong>Our services are prepaid.<\/strong><\/p><p>Here are our payment terms:<\/p><ul><li>PayPal<\/li><li>Transfer of funds (Etransfer)<\/li><li>Credit card<\/li><li>Debit card<\/li><\/ul><p>As for your credit or debit cards, please make sure you have them with you when you make the request, and that they are cleared at the bank. <strong>For your security, we will ask you for the tracking number <\/strong><strong>indicated in the subject line of your e-mail<\/strong>.
